Simplify top-level table of contents and create the user guide which
includes the application development primer and other guides and
refereces instead of having them at the top-level.
Also move glossary section away from top-level TOC and remove broken
search link.

Add cross-referenced information on the 96b_carbon and
96b_carbon_nrf51 pages which disambiguates between the two "boards".
Also describe how to flash 96b_carbon_nrf51 with
samples/bluetooth/hci_spi and 96b_carbon with samples/bluetooth/ipsp
to support a Bluetooth HCI stack on 96Boards Carbon (the physical
board).
While we're here, make the documentation page for 96b_carbon match the
format in doc/templates/board.tmpl.

Add a sample application that allows a Zephyr-based Bluetooth
controller to interface with an HCI driver via SPI. This sample
implements the same BT SPI protocol already as Zephyr's HCI SPI
driver.
Currently, the sample only supports the legacy SPI API.
Provide a single configuration file, avoiding board-specific
files. Some board-specific configuration information must be provided
via other means:
- CONFIG_BT_CONTROLLER_TO_HOST_SPI_DEV_NAME
- CONFIG_BT_CONTROLLER_TO_HOST_SPI_IRQ_DEV_NAME
- CONFIG_BT_CONTROLLER_TO_HOST_SPI_IRQ_PIN

Commit 7cb8a16c86 ("doc: restructure application primer") removed
the section documenting the build system support for third party
libraries. Restore this section making a few editorial changes to
ensure the text sits well in its surroundings.

The headings on some .rst files were not following the expected
heading order of using # for h1, * for h2, = for h3, and - for h4
This patch fixes that, and the doc/templates/*.tmpl files created
for folks to use as templates for creating board and sample docs.

This will replace the current goal of 'make qemu' with 'make run' and
moves Qemu handling into its own file and into the boards instead of
being architecture specific.
We should be able to add new boards that support some other type of
emulation (by adding scripts/Makefile.<emu type>) and allow the board to
define their own options for the use type of emulation.
'make qemu' will still work, however it will be deprecated, starting
with this commit it is recommended to use 'make run'.

Use the existing debugserver target also for Qemu debugging. Qemu
should be maintained as one of many emulation/simulations platforms and
emulation should be abstracted in the Makefiles and not tied to Qemu.
qemugdb will still work, it is however being deprecated.

In addition to providing documentaton, we also modify the static_lib
example to use outputexports instead of the double-include
Makefile.toolchain trick. For this to work well we need to fix a tiny
bug in the dependency handling in Makefile.inc.

Relocates material that outlines the structure of a Zephyr application
so that it appears at the beginning of the "Application Development
Primer". (i.e. The primer now tells the reader the file structure of
an application, then explains how to create these files.)
Revises the description of the MDEF file to make it a bulleted item
(like the other file types are) and improve readability.

Restructured the guide to follow a basic three step process: Set up, develop,
build. All information that is independent of the OS selection was taken out of
of the OS related documents and placed in the general set up sections.
A new section, Developing Zephyr Applications, was added with links to all the
development relevant section of the documentation.
The content in local_development.rst was split between the setting_up.rst,
installation_linux.rst and installation_mac.rst files as appropriate.
This new structure provides a much better flow for developers beginning their
Zephyr develpment.
Applied all of Inaky's suggestions in the refactored content.
Minor markup fixes and language edits were also perfomed during the refactoring.
Fixed a cross-reference that was broken during the refactor.
Applied all the feedback from the first trial run.
Applied the feedback provided by David Kinder from TCS.

Setting up new platforms to handle emulation, and make them the only
ones able to run on QEMU from the Makefile "qemu" target to avoid
confusion with other platforms. We have now platform qemu_x86 and
platform qemu_cortex_m3, also modification to the sanity checks to have
qemu support only on those platforms

To make the documentation readable from the source I want to get rid of the
substitutions for the project name an code name. This does not add any values
and makes it unreadable when looking at the text files directly. It also causes
some issues when people use those without actually knowing what they represent,
resulting in some weird and redundant language.

Fixes the following error when generating the apps_microkernel documentation.
.../apps_microkernel.rst:57:ERROR: Error in "code-block" directive:
unknown option: "lineos".
The option was supposed to be "linenos". The generated HTML document now
properly displays the sample MDEF file contents.
