The header scheme for the IPM_CAVS_IDC driver changed and this legacy
platform (which is really a very close cousin of intel_adsp/cavs_v15)
broke. Fix things up. Longer term we should unify the two.

Now that device_api attribute is unmodified at runtime, as well as all
the other attributes, it is possible to switch all device driver
instance to be constant.
A coccinelle rule is used for this:

This set of functions seem to be there just because of historical
reasons, stemming from Kbuild. They are non-obvious and prone to errors,
so remove them in favor of the `_ifdef()` ones with an explicit
`CONFIG_` condition.

Of course IPM drivers now provide their device instance. There are 2
drivers using IPM callbacks as well, so they get the imp device instance
pointer now through the callback.

The second paramater to IRQ_CONNECT for ipm_mhu_irq_config_func_0 should
be passing the priority, instead it passed the IRQ number. Fix this to
pass priority (which matches ipm_mhu_irq_config_func_1).

Since idc_read/idc_write are used outside of the driver we can't use the
DT_INST_... form (that utilize DT_DRV_COMPAT) of the macro's in
ipm_cavs_idc_priv.h. Use DT_INST(0, intel_cavs_idc) explicitly in the
header to fix the build issue.

This adds a rather primitive driver for use with the Intra-DSP
Communication (IDC) on the DSP on certain Intel SoCs. The IDC
generates interrupts from one core to another by writing to
certain registers. This is also being utilized as
the scheduler IPI since it can interrupt other cores.

Remove the
# Omit prompt to signify a "hidden" option
comments that appear on some symbols. They seem to have been copy-pasted
at random, as there are lots of promptless symbols that don't have them
(that's confusing in itself, because it might give the idea that the
ones with comments are special in some way).
I suspect those comments wouldn't have helped me much if I didn't know
Kconfig either. There's a lot more Kconfig documentation now too, e.g.
https://docs.zephyrproject.org/latest/guides/kconfig/index.html.
Keep some comments that give more information than the symbol having no
prompt.
Also do some minor drive-by cleanup.

These calls are not accessible in CI test, nor do they get built on
common platforms (in at least one case I found a typo which proved the
code was truly unused). These changes are blind, so live in a
separate commit. But the nature of the port is mechanical, all other
syscalls in the system work fine, and any errors should be easily
corrected.

MHU (Message Handling Unit) enables software to raise interrupts to
the processor cores. It is enabled in SSE 200 subsystems.

Update the files which contain no license information with the
'Apache-2.0' SPDX license identifier. Many source files in the tree are
missing licensing information, which makes it harder for compliance
tools to determine the correct license.
By default all files without license information are under the default
license of Zephyr, which is Apache version 2.

Update reserved function names starting with one underscore, replacing
them as follows:
'_k_' with 'z_'
'_K_' with 'Z_'
'_handler_' with 'z_handl_'
'_Cstart' with 'z_cstart'
'_Swap' with 'z_swap'
This renaming is done on both global and those static function names
in kernel/include and include/. Other static function names in kernel/
are renamed by removing the leading underscore. Other function names
not starting with any prefix listed above are renamed starting with
a 'z_' or 'Z_' prefix.
Function names starting with two or three leading underscores are not
automatcally renamed since these names will collide with the variants
with two or three leading underscores.
Various generator scripts have also been updated as well as perf,
linker and usb files. These are

Some more were added since the cleanup pass in June 2018. See e.g.

These changes were obtained by running a script created by
Ulf Magnusson <Ulf.Magnusson@nordicsemi.no> for the following
specification:
1. Read the contents of all dts_fixup.h files in Zephyr
2. Check the left-hand side of the #define macros (i.e. the X in
#define X Y)
3. Check if that name is also the name of a Kconfig option
3.a If it is, then do nothing
3.b If it is not, then replace CONFIG_ with DT_ or add DT_ if it
has neither of these two prefixes
4. Replace the use of the changed #define in the code itself
(.c, .h, .ld)
Additionally, some tweaks had to be added to this script to catch some
of the macros used in the code in a parameterized form, e.g.:
- CONFIG_GPIO_STM32_GPIO##__SUFFIX##_BASE_ADDRESS
- CONFIG_UART_##idx##_TX_PIN
- I2C_SBCON_##_num##_BASE_ADDR
and to prevent adding DT_ prefix to the following symbols:
- FLASH_START
- FLASH_SIZE
- SRAM_START
- SRAM_SIZE
- _ROM_ADDR
- _ROM_SIZE
- _RAM_ADDR
- _RAM_SIZE
which are surprisingly also defined in some dts_fixup.h files.
Finally, some manual corrections had to be done as well:
- name##_IRQ -> DT_##name##_IRQ in uart_stm32.c
